Simply contact us and request the "Opt-Out" form found in our Privacy Policy Brochure. Fill out the form, sign your name, and mail it back to our offices (see address below) for processing. But please understand, we will continue to share your information with affiliated and non-affiliated parties who assist us in providing service to you or to non-affiliated parties in special required situations such as: lawsuits, state association and board reviews, merger or purchase discussions or federal agency processes such as the Internal Revenue Service as permitted or required by law.
